Why Does Sciatica Keep Coming Back?
Your Non-Surgical Options

When leg pain returns, a clearer plan starts with better questions.

Why can sciatica return?

A good week, then another difficult drive. A comfortable morning, then leg pain that interrupts dinner. When symptoms keep returning, the question becomes: “Why am I back here again?”

Sciatica describes nerve-related symptoms, not a single diagnosis. A herniated disc or spinal narrowing can irritate nerve tissue. Symptoms may include pain traveling down a leg, tingling, numbness, or weakness. Back pain alone does not necessarily mean sciatica.

Symptoms can settle and later recur. That does not, by itself, tell you whether the original problem has changed or whether something else needs attention. The useful next step is to reassess the pattern rather than assume every episode has the same explanation.

Start with the pattern, not a treatment menu

Before choosing a treatment, organize the details of your experience. Bring these questions to your appointment:

  • Where does the discomfort start, and how far down the leg does it travel?
  • What has changed since the last episode?
  • Which activities are difficult now: driving, walking, sleeping, or working?
  • What have you already tried, and what happened afterward?
  • Have you noticed weakness, numbness, or changes in bladder or bowel function?

An evaluation can help determine whether the symptoms fit sciatica and whether further medical assessment is needed. Imaging is not automatic: NICE recommends considering it in specialist care when the result is likely to change management.

What non-surgical care can include

Guideline-supported care includes education, staying active within your abilities, and exercise tailored to your needs. Manual therapy may be considered as part of a plan that includes exercise, rather than as a standalone solution.

Ask for a plan you can explain in plain language: what you will do at home, what each office visit is intended to accomplish, and when progress will be reviewed. Choose meaningful goals, such as getting through a workday or returning to a comfortable walk.

Our Shelton office offers non-surgical spinal decompression, MLS Laser Therapy, and other services. Availability does not mean a treatment is right for every case. Evidence and suitability differ by treatment; NICE recommends against traction for low back pain with or without sciatica. Ask how any proposed device-based treatment compares with an exercise-based approach, including its limitations, cost, and alternatives.

No treatment should be presented as a guaranteed disc repair or permanent cure. Persistent or worsening symptoms may need specialist evaluation, and some people ultimately require surgical care.

Make your next step manageable

Keep a brief record of the activities that matter most to you and how symptoms affect them. Bring previous reports and a list of treatments you have tried. Instead of asking only “Will this make the pain stop?”, also ask:

  • What findings support the working diagnosis?
  • What improvement should we look for, and when?
  • What would make us change the plan or seek another opinion?

These questions turn a treatment discussion into a shared decision with clear expectations.

When not to wait for an office visit

Seek emergency care for new bladder or bowel control problems, difficulty passing urine, numbness around the groin or buttocks, or severe or worsening weakness or numbness in both legs. Symptoms affecting both sides also need urgent assessment. New or progressive leg weakness warrants prompt medical evaluation.
